Health Assessment Questionnaires app for iPhone and iPad

This application illustrates the health assessments of the well-being. It navigates three questionnaires of the three elements of health:
1.Mental Health
2.Spiritual Health
3.Body Health
It asks the operator questions about their behavior, feeling, sensing, and consumptions of nourishments.

The operator can answer to six options:
1.Rare
2.Seldom
3.Sometimes
4.Often
5.Regular
6.Always
The app translates the answers into measurements of the 7 categories of health.

The operator can retake the assessment as many times as possible and whenever they desire to assess their health.
